Job Summary

The PCP independently provides care for patients with acute and chronic illnesses encountered in the older adult patient. They take full accountability for patient care and outcomes, and will seek consultation when needed while staying involved in detailed patient care. The role requires engagement with hospitalists for hospitalized patients, leadership of a care team (medical assistant, care facilitator, care coordinator) for office-based patients, and collaboration with transitional-care and case-management resources for homebound or non-office visits. The PCP will actively participate in center governance, recruitment of patients and providers, and coverage for other providers as needed. The position emphasizes leadership, accountability for outcomes, and adherence to regulatory standards, quality metrics, and practice guidelines. Skills emphasized include availability and accessibility to patients, patient-centered service, evidence-based medicine, physician leadership, quality improvement, and the ability to influence teams toward high-quality outcomes. Proficiency in Microsoft Office and willingness to travel for training and recruiting is required. The role supports ongoing professional development (MOC, CME) and requires a current medical license, DEA license, and BLS credentialing. Required Qualifications

  • MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty is required
  • Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
  • Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
  • Must have a current DEA number for schedule II-V controlled substances
  • Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment
  • Demonstrated availability to travel locally for market needs and potential national travel for initial training and recruitment purposes
  • Fluency in English, verbal and written; other language requirements may be specified by center
  • MD or DO degree; active license; board eligibility/board certification as applicable
