Job Summary
Provide comprehensive primary care for older adults with acute and chronic illnesses, lead a care team (medical assistant, care facilitator, care coordinator) and coordinate in-hospital, SNF/LTC, or homebound care as needed; manage a patient panel up to 450, ensure accountability for outcomes and quality metrics, engage in risk and quality management, and participate in recruiting patients and providers; requires MD/DO with licensure, board certification preferred, DEA, BLS, and a focus on evidence-based, patient-centered, value-based care; includes collaboration with hospitalists, APPs, and center leadership; strong leadership and communication skills essential.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred. Board Eligibility is required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment
