Job Summary
As a Primary Care Physician (PCP), you will provide independent care for older adult patients with acute and chronic illnesses, taking accountability for patient outcomes while leading a care team. Responsibilities include building trust with patients, acting as a health coach, setting and following up on health goals, managing clinical care, and engaging with hospitalists. The role demands a commitment to continuous improvement in patient care and requires strong leadership, service orientation, and evidence-based practice. Candidates must have an MD or DO in relevant specialties, a current state medical license, and board eligibility to contribute to the transformation of healthcare for seniors.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Must be able to obtain a State Medical License
- Board Eligibility required
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification required within first 90 days of employment
Desired Qualifications
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred
- Ability and willingness to travel locally and nationally for training and recruitment purposes
- Proficient in Microsoft Office Suite (Word, Excel, PowerPoint, Outlook)
