Job Summary
The Primary Care Physician (PCP) is accountable for outcomes and provides care for patients with acute and chronic illnesses, managing up to 450 patients. They engage patients as health coaches, simplify care processes, and lead care teams towards achieving health goals. Duties include assessment, diagnosis, treatment, care coordination, and compliance with healthcare guidelines. The position requires proficiency in Microsoft Office, effective communication skills, and the ability to work both independently and collaboratively. Candidates must hold a DO or MD and maintain certification and licensure.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Current active State Medical License in the state(s) of practice
- Board Eligibility is required
- Current DEA number for Schedule II-V controlled substances
- Basic Life Support (BLS) certification required within first 90 days of employment
Desired Qualifications
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty preferred
- Fluency in other languages as per job center requirements
