Primary Care Physician

Conduct geriatric assessments, medical histories, and physical exams to diagnose and treat patients in ambulatory, acute, nursing home, and home settings. Develop comprehensive plans of care, manage referrals across specialties, and document progress notes adhering to ChenMed standards. Order and interpret diagnostic studies, administer medications, and write discharge orders while collaborating with multidisciplinary teams to transition patients to lower levels of care. Participate in clinical rounds, conferences, and risk management programs to improve quality and safety. This role supports ChenMed's mission to transform healthcare for seniors by providing direct patient advocacy and education.
