Job Summary
Conduct geriatric assessments, medical histories, and physical exams to diagnose and treat patients in ambulatory, acute, nursing home, and home settings. Develop comprehensive plans of care, manage specialty referrals, and document progress notes adhering to ChenMed standards. Independently order laboratory studies, medications, and treatments while collaborating with multidisciplinary teams to optimize resource utilization and facilitate patient transitions. Participate in clinical rounds, conferences, and quality management programs to improve care processes and outcomes.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
Desired Qualifications
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty
