Job Summary
Provide care for older adults with acute and chronic illnesses, taking full accountability for patient outcomes for a panel of up to 450 patients. Act as a health coach to help patients set and pursue short- and long-term health goals, collaborating with the patient to implement action plans. Lead and coordinate care teams (care promoter, care facilitator, care coordinator) across office, hospital, skilled nursing, long-term care, and homebound settings; engage hospitalists when patients are hospitalized; manage patient assessment, diagnosis, treatment, education, health promotion and care coordination; participate in risk, quality, and performance improvement activities; recruit patients and providers as needed; supervise APPs and ensure compliance with guidelines and procedures; maintain independence and collaboration with healthcare professionals; travel locally for market activities and nationally for training or recruitment as required; fluency in English; willingness to work evenings, weekends, and holidays; MD/DO licensed to practice in the applicable state; board eligibility or certification in Internal Medicine, Family Medicine or Geriatrics; BLS certification required.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred
- Current State Medical License for the state(s) in which he/she will be working
- Active DEA number for schedule II-V
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required w/in first 90 days of employment.
