Job Summary
The Primary Care Physician (PCP) at ChenMed provides comprehensive care for up to 450 patients with acute and chronic illnesses, demonstrating accountability for patient outcomes and engaging in health coaching to help patients achieve health goals. The PCP collaborates with care teams and leads care coordination efforts, ensuring compliance with guidelines and quality management programs. Required qualifications include an MD or DO, a valid state medical license, and board eligibility. Key responsibilities also include effective patient communication and participation in risk management initiatives.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty required.
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working.
- Board Eligibility is required.
- Must have a current DEA number for schedule II-V controlled substances.
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross required within first 90 days of employment.
Desired Qualifications
- Board certification in Internal Medicine, Family Medicine, Geriatrics or similar specialty is preferred.
- Fluency in other languages may be required based on the center.
