Job Summary
Conduct senior-focused primary care for an established patient panel, seeing 14-17 patients daily with 45-minute appointments. Deliver hands-on clinical care inside the exam room, addressing physical health while evaluating social determinants of health. Manage chronic conditions, screenings, minor injuries, and wellness visits within a value-based model. Work onsite Monday through Friday, 8:00am to 5:00pm, reporting to the Associate Medical Director. Complete required TB screening and maintain unrestricted state and DEA licenses.
Required Qualifications
- Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license
- Willingness to complete a TB Screening
Desired Qualifications
- Minimum of two+ years of experience
- 1+ years of experience managing Medicare Advantage panel of patients
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
