Job Summary
Conduct proactive, preventive care for seniors, including wellness visits, physical exams, chronic condition management, screenings, and minor injury treatment. Address social determinants of health affecting patient outcomes while managing a panel of 14-17 patients daily. Deliver longer 45-minute appointments with 100% clinical focus on direct patient care, including general administrative tasks related to patient-facing services. Report to the Associate Medical Director at the Conviva Delray Beach Center. Complete required TB screening and maintain valid, unrestricted medical and DEA licenses.
Required Qualifications
- Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of desired employment (or willing to obtain)
- Current, unrestricted DEA license
- Willingness to complete a TB Screening
Desired Qualifications
- Minimum of two+ years of experience
- 1+ years of experience managing Medicare Advantage panel of patients
- Knowledge of Medicare guidelines and coverage
- Knowledge of HEDIS quality indicators
