Job Summary
Conduct proactive, preventive care for seniors, including wellness visits, physical exams, chronic condition management, screenings, minor injury treatment, and more. Manage an average of 15 patients daily with 45-minute authorized appointments while working onsite at the Palm Coast clinic Monday through Friday from 8:00am to 5:00pm. Address the social, emotional, behavioral, and financial needs impacting patient well-being within a value-based approach focused on whole health.
Required Qualifications
- Doctor of Medicine or Osteopathic Medicine
- Board certified or board eligible in Geriatric Medicine, Family Medicine or Internal Medicine
- Valid, unrestricted license in the state of Florida, or able to obtain one
- Current, unrestricted DEA license
- Screened for TB
- Minimum of two+ years of experience
- 1+ years of experience managing Medicare Advantage panel of patients
- Associate who speaks with a member in a language other than English must take a language proficiency assessment, provided by an outside vendor, to ensure competency
- Applicants will be required to take the Interagency Language Rating (ILR) test as provided by the Federal Government
- You will be onsite at the Palm Coast clinic
- You will be on-site
- Monday-Friday 8:00am-5:00pm
- See an average of 15 patients per day
- Longer authorized appointments (45 minutes)
