Job Summary
Deliver comprehensive outpatient care to adult and senior patients while diagnosing and managing moderately to complex medical conditions. Maintain accurate medical records, coordinate referrals with specialists, and participate in clinical quality improvement programs. Exercise independent clinical judgment and supervise Advanced Practice Providers in a collaborative, patient-centered environment. Work fully onsite at a CenterWell clinic in Frankfort or Lexington, KY, with a standard Monday–Friday schedule from 8:00 AM to 5:00 PM and quarterly limited on-call coverage by phone.
Required Qualifications
- MD or DO from an accredited medical school
- Active, unrestricted medical license in the state of practice
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Fellowship
- Must be willing to participate in a quarterly limited on-call coverage by phone
- Proficiency with electronic health records and digital documentation
- 2 years' experience managing high-acuity geriatric patients
- Supervise Advanced Practice Providers (NPs) in your assigned clinic
- Must be able to lift 50 lbs
- Must be available for weekend shifts
- Valid driver's license
Desired Qualifications
- Specialty training in an Outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics
- Experience working in a Value-Based Care settings
- Proficiency with electronic health records Athena Health
