Job Summary
Deliver comprehensive outpatient care to adult and senior patients, diagnosing and managing moderately to complex medical conditions while maintaining accurate medical records. Coordinate referrals, collaborate with specialists, and exercise independent clinical judgment in patient management. Participate in clinical quality improvement programs and work with interdisciplinary teams to ensure holistic care. Supervise Advanced Practice Providers (NPs) and contribute to strategic programs and innovations in care delivery. Report to the Associate Medical Director for the TN market with a full-time, onsite schedule at CW Bellevue: Nashville, TN. This role offers a collaborative, patient-centered environment focused on whole-person care with minimal call responsibilities.
Required Qualifications
- MD or DO from an accredited medical school
- Active, unrestricted medical license in the state of practice
- Board Certification in Family Medicine, Internal Medicine, or Geriatric Medicine
- 2 years' experience managing high-acuity geriatric patients
- Commitment to improving patient experience and outcomes
- Supervise Advanced Practice Providers (NPs)
- TB screening program participation
Desired Qualifications
- Specialty training in an Outpatient clinical setting in Primary Care, Family Medicine, Internal Medicine, or Geriatrics
- Experience working with senior populations or in Value-Based Care settings
- Proficiency with electronic health records Athena Health and digital documentation
- Outpatient clinical setting
