Job Summary
Deliver comprehensive primary care at home and in senior communities in Saint Louis, MO, with visits to patients, coordination of care with care teams, and engagement with facilities and community organizations. Build patient panels, analyze the local healthcare landscape, and coordinate with interdisciplinary teams to achieve optimal outcomes. Travel to Denver for initial and ongoing training; relocation assistance available. Schedule is Monday–Friday with flexible total days; benefits include health/dental/vision coverage, 401k, PTO and holidays, CME allowance, and licensing/liability coverage. Licensure in Missouri required; bilingual preferred but not required. Bloom Healthcare is employee-owned and emphasizes home-based, high-touch care for homebound patients.
