Job Summary
Provide comprehensive, longitudinal patient-centered care in an outpatient setting while collaborating with an experienced multidisciplinary team. Perform patient evaluations, medical histories, and physical examinations to diagnose and manage acute illnesses and chronic conditions. Develop individualized treatment plans emphasizing preventive care and wellness, then order, interpret, and follow up on laboratory studies and diagnostic imaging. Prescribe medications consistent with clinical guidelines and coordinate transitions of care, including hospital follow-up. Manage approximately 20 patients daily while partnering with physicians and advanced practice providers.
