Primary Care Physician

Deliver high-quality, patient-centered primary care through in-person and virtual options, including same-day or next-day appointments available seven days a week. Manage risk-stratified patient populations with support from RN Care Managers and embedded behavioral health specialists, including onsite social workers and psychiatrists. Coordinate seamless transitions for patients requiring inpatient care via collaborative hospitalist support while utilizing on-site laboratory and pharmacy services. Serve diverse populations through Federally Qualified Health Center locations offering sliding-scale fees and participate in community health initiatives and population health improvement projects.
