Primary Care Physician

Conduct comprehensive primary care services in a team-based, integrated model featuring embedded behavioral health support and RN care managers. Manage risk-stratified patient populations while coordinating seamless transitions for those requiring collaborative hospitalist support. Deliver flexible in-person and virtual care options, including same-day appointments seven days a week, utilizing on-site laboratory and pharmacy services. Serve diverse populations through Patient-Centered Medical Homes with sliding-scale fees and actively participate in community health initiatives and population health improvement projects.
