Primary Care Physician

Deliver high-quality, patient-centered primary care through in-person and virtual options, including same or next-day appointments seven days a week. Manage risk-stratified patient populations with support from RN Care Managers and onsite behavioral health specialists, including social workers, psychiatrists, and advanced practice nurses. Coordinate seamless transitions for patients requiring inpatient care via collaborative hospitalist support. Serve diverse populations through Federally Qualified Health Center locations offering sliding-scale fees and participate in community health initiatives and population health improvement projects.
