Primary Care Physician

Deliver high-quality, patient-centered primary care through in-person and virtual options, including same or next-day appointments seven days a week. Manage risk-stratified patient populations with support from RN Care Managers and collaborate seamlessly with embedded behavioral health providers, including social workers, psychiatrists, and advanced practice nurses. Coordinate care for patients requiring inpatient services through collaborative hospitalist support while utilizing on-site laboratory and pharmacy resources. Serve diverse populations through Patient-Centered Medical Homes with sliding-scale fees and actively participate in community health initiatives and population health improvement projects.
