Primary Care Physician

Conduct comprehensive primary care services including in-person and virtual appointments, same-day scheduling, and 7-day availability. Manage risk-stratified patient populations with support from RN Care Managers and embedded behavioral health specialists. Coordinate seamless transitions for patients requiring inpatient care via collaborative hospitalist support. Utilize on-site laboratory and pharmacy resources to ensure care efficiency. Serve diverse populations through Patient-Centered Medical Homes with sliding-scale fees and participate in community health initiatives and population health improvement projects.
