Deliver high-quality, patient-centered primary care at the Manahawkin Health Park location, offering both in-person and virtual appointments seven days a week. Manage risk-stratified patient populations with support from RN Care Managers and embedded behavioral health specialists including social workers and psychiatrists. Coordinate seamless transitions for patients requiring inpatient care through collaborative hospitalist support and utilize on-site laboratory and pharmacy services. Participate in community health initiatives, wellness programs, and population health improvement projects to advance health equity. Join a multi-specialty team within AtlantiCare's multi-specialty group, contributing to the Vision 2030 initiative focused on digital health and interdisciplinary integration.
