Job Summary
Provide independent primary care for older adults with acute and chronic illnesses, managing assessment, diagnosis, treatment, and care coordination for up to 450 patients. Act as a health coach by setting goals, partnering with patients on behavior changes, and simplifying care plans to improve outcomes. Lead a care team including medical assistants and coordinators, while engaging with hospitalists, transitional care teams, and specialists as needed. Maintain strict adherence to quality metrics, regulatory compliance, and center leadership goals, including recruiting and covering for absent providers. Work evenings, weekends, and holidays as required, with local and occasional national travel for training and recruiting.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
- Fluency in English, verbal and written
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
- This job requires use and exercise of independent judgment
- Flexible to work evening, weekends and/or holidays as needed
