Job Summary
Provide comprehensive assessment, diagnosis, treatment, and care coordination for older adult patients with acute and chronic illnesses, managing a panel of up to 450 individuals. Act as a health coach by partnering with patients to achieve short and long-term health goals while maintaining accountability for clinical outcomes and cost-effectiveness. Lead care teams including medical assistants and coordinators, and engage with hospitalists and transitional care resources for patients unable to visit the office. Collaborate with specialists and center leadership to meet performance metrics on admissions, appointment slots, and quality closures. Cover for other providers as needed and recruit additional patients and staff. Work evenings, weekends, and holidays, and travel locally or nationally for training and recruiting.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
- Fluency in English, verbal and written
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
- Flexible to work evening, weekends and/or holidays as needed
Desired Qualifications
