Job Summary
Provide independent primary care for older adults with acute and chronic illnesses, managing assessment, diagnosis, treatment, and care coordination for up to 450 patients. Act as a health coach to help patients achieve behavioral goals while adhering to value-based metrics such as CGCAHPS, clinical gaps, and cost measures. Lead a care team including medical assistants and coordinators, engage with hospitalists and transitional care providers for out-of-office patients, and collaborate with managers on risk and quality management programs. Maintain strict regulatory compliance and champion a positive culture of accountability. Work evenings, weekends, and holidays as needed, with local travel for training and recruiting.
Required Qualifications
- MD or DO in Internal Medicine, Family Medicine, Geriatrics or similar specialty
- Must be able to obtain a State Medical License or already have a current, active State Medical License for the state(s) in which he/she will be working
- Board Eligibility
- Must have a current DEA number for schedule II-V controlled substances
- Basic Life Support (BLS) certification from the American Heart Association (AMA) or American Red Cross
- Fluency in English, verbal and written
- Ability and willingness to travel locally as needed in their market, if applicable, nationally for initial training (2 weeks) and then occasionally regionally and nationally for recruiting or training purposes
