Job Summary
Primary Care physician (PCP) provides comprehensive medical care to PACE Organization participants within a nursing home setting as part of an interdisciplinary team. Responsibilities include conducting initial and ongoing participant assessments, developing and implementing care plans, coordinating with medical specialists, admitting to hospital when needed, overseeing use of inpatient care, participating in IDT meetings, and contributing to quality assurance and policy development. Requires certification as MD or DO, CDS and DEA, current BLS, and board certification in Internal Medicine or Family Practice with geriatrics preferred; minimum one year of experience with frail or elderly populations; valid driver’s license and ability to travel regionally; strong communication, leadership, and organizational skills; proficiency with Microsoft Office and ability to operate in a virtual, collaborative environment.
Required Qualifications
- MD or DO
- CDS Certification
- DEA Registration
- Current BLS
- Board Certification Internal Medicine or Family Practice
- Board Certification in Geriatrics preferred
- Minimum of one (1) year working with the frail or elderly population
- Valid driver’s license and means of transportation
- Ability to work with minimal supervision and exercise independent judgment
- Interpersonal and communication skills suitable for diverse audiences
- Proficiency with Microsoft Office suite (MS Word, Excel, PowerPoint)
- Willingness to travel within Centers geographic region
