Job Summary
Provide primary care services to residents in skilled nursing and long-term care facilities, including performing physical exams, diagnosing conditions, and developing individualized care plans. Order and interpret diagnostic tests, prescribe medications, and collaborate with interdisciplinary teams while educating patients and families on treatment goals. Ensure a safe, respectful environment for residents while maintaining a minimum of 6 months of long-term care experience. Full-time or part-time roles available in Logansport, IN with a salary ranging from $30,000 to $200,000.
Required Qualifications
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific license in good standing in Indiana
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Strong communication, clinical, and critical thinking skills
- Must be able to lift a minimum of 20 pounds
- Must be able to stand / walk / work on your feet up to 6-8 hours per day
