Job Summary
Advanced Practice Clinician role providing comprehensive primary care in skilled nursing and senior living communities. Responsibilities include routine rounding, proactive chronic disease management, care coordination across interdisciplinary teams, and close collaboration with facility partners. Licensure as NP or PA required. Emphasis on preventive, whole-person care, resident-centered approaches, and collaboration with physicians, behavioral health specialists, care coordinators, and facility staff. Required Qualifications
- Current NP or PA licensure in the state of practice

- Strong collaboration skills with facility partners
