Job Summary
Provide comprehensive primary care services within skilled nursing and senior living communities as an Advanced Practice Clinician (Nurse Practitioner or Physician Assistant). Responsibilities include routine rounding, proactive chronic disease management, coordination of care across interdisciplinary teams, and close collaboration with facility partners. Emphasizes preventive care, improved quality outcomes, and a better experience for patients, families, and care teams. Supported by AI-enabled tools to enhance clinical decision-making and streamline workflows, with a value-based approach and opportunities for professional growth. Required Qualifications
- Current NP or PA licensure in the state of practice
- Previous professional patient care experience
- Experience in geriatrics, family medicine, or internal medicine preferred
- Exceptional patient-centered service skills, including compassionate communication, active listening, professionalism, and the ability to build trusting relationships with residents, families, and facility staff
- Strong collaboration skills with the ability to work effectively across interdisciplinary teams
