Primary Care Nurse Practitioner or Physician Assistant

Deliver longitudinal, holistic primary care to homebound seniors and medically complex patients within defined panels. Diagnose and manage chronic and acute conditions while addressing social determinants of health such as housing stability and nutrition. Conduct specialty visits including wound care and post-hospital transitional care, utilizing integrated data systems to coordinate timely post-discharge visits and reduce readmissions. Complete comprehensive evaluations for medication reconciliation, fall risk assessments, and advance care planning, collaborating closely with interdisciplinary teams to ensure continuity. Practice independently with full support from MD leadership and advanced analytics. Monday through Friday schedule with manageable volumes of 8-12 patients daily and light call rotation.
