Primary Care Nurse Practitioner or Physician Assistant

Primary Care NP/PA delivering longitudinal, home- and community-based primary care to a defined patient panel. Diagnose, manage, and treat chronic and acute conditions with support from an interdisciplinary team; address social determinants of health; conduct wound care, post-hospital transitional care, and condition-specific assessments; perform comprehensive evaluations including medication reconciliation, fall risk assessments, and health education for patients and caregivers. Employ Bloom’s integrated data ecosystem to track outcomes, coordinate timely post-discharge visits within 72 hours, and collaborate with internal/external care teams to reduce readmissions and stabilize high-risk patients. Schedule features include Monday–Friday with 8–12 patients per day, light call rotation, weekends off, robust benefits, PTO, 401(k), and licensing/malpractice support. Eligible candidates are NPs or PAs with a master’s degree and active Texas licensure who are passionate about serving seniors and medically complex populations; dedicated to mission-driven, relationship-based care; and prepared to practice independently within a team-based framework.
