Job Summary
Primary Care NP/PA deliver longitudinal, home- and community-based primary care to homebound seniors. Provide autonomous, relationship-based care, diagnose/manage chronic and acute conditions, address social determinants of health, perform wound care and post-hospital transitional care, coordinate with MD leadership and interdisciplinary teams, and leverage Bloom’s data ecosystem to track outcomes and ensure timely post-discharge visits (within 72 hours). Requires NP or PA with master’s degree and active Colorado licensure; compensation $100k–$125k annually with strong benefits and supportive team.
Required Qualifications
- Master's degree (NP or PA)
- Active Colorado licensure as NP or PA
- Experience in primary care or home-based care preferred
- Ability to diagnose/manage chronic and acute conditions
- Ability to conduct wound care and post-hospital transitional care
- Comfort with social determinants of health discussions
- Ability to coordinate with interdisciplinary teams
- Experience with health data systems or analytics is a plus
