Primary Care Nurse Practitioner or Physician Assistant

Deliver longitudinal, holistic primary care to homebound seniors and medically complex patients in the Dallas/Fort Worth area. Diagnose and manage chronic and acute conditions while addressing social determinants of health such as housing stability and nutrition. Conduct specialty visits including wound care and post-hospital transitional assessments, utilizing integrated data systems to coordinate timely post-discharge visits and reduce readmissions. Complete comprehensive evaluations for medication reconciliation, fall risk, and advance care planning with full support from an interdisciplinary team. Practice independently with manageable volumes of 8-12 patients daily, featuring light call rotation and weekends off.
