Job Summary
Provide on-site primary care to residents in long-term care facilities. Perform physical exams and medical history assessments, diagnose and treat chronic and acute health conditions, order and interpret diagnostic tests and lab results, develop and adjust individualized care plans, prescribe medications and monitor treatment effectiveness, collaborate with facility staff and interdisciplinary teams, educate patients and families about treatment options and care goals, and ensure a safe, respectful, and supportive environment for residents. Requires active NP/PA license with state licensure in good standing in Texas; minimum 6 months of long-term care or skilled nursing experience. Full-time schedule in New York facilities.
Required Qualifications
- Active and unrestricted Nurse Practitioner, Physician Assistant, or Physician license
- State-specific license in good standing in Texas
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Strong communication, clinical, and critical thinking skills
- Passion for elder care and person-centered healthcare
