Job Summary
Provide primary care services to residents in skilled nursing and long-term care facilities, including performing physical exams, medical history assessments, and diagnosing and treating chronic and acute health conditions. Order and interpret diagnostic tests and lab results, develop individualized care plans, and prescribe medications while monitoring treatment effectiveness. Collaborate with facility staff and interdisciplinary teams to educate patients and families about treatment options and care goals, ensuring a safe, respectful, and supportive environment. Requires an active, unrestricted North Carolina Nurse Practitioner license with at least six months of long-term care experience, plus the ability to lift 20 pounds and stand for up to eight hours daily.
Required Qualifications
- Active and unrestricted Nurse Practitioner
- State-specific license in good standing in North Carolina
- Minimum 6 months of experience in long-term care or a skilled nursing facility
- Strong communication, clinical, and critical thinking skills
- Passion for elder care and person-centered healthcare
- Must be able to lift a minimum of 20 pounds
- Must be able to stand / walk / work on your feet up to 6-8 hours per day
