Primary Care Nurse Practitioner

Job Summary
Conduct in-person, virtual, or in-home primary care visits for adult members aged 18 and up, including routine physical exams, screenings, and wellness assessments. Develop individualized care plans, educate patients on preventive strategies and chronic disease management, and coordinate referrals with specialists as needed. Maintain accurate patient records and collaborate within a multidisciplinary team to improve the member experience. This per diem role starts in the DMV area with potential growth into part-time or full-time positions, requiring an active FNP or AGPCNP license and at least two years of direct adult primary care experience.
Required Qualifications
- Active license in D.C., Maryland, or Virginia as one of the following: Family Nurse Practitioner (FNP)
- Active license in D.C., Maryland, or Virginia as one of the following: Adult-Gerontology Primary Care Nurse Practitioner (AGPCNP)
- Must have professional liability insurance or willingness to obtain it
- Must be licensed and live in DMV Area (D.C., Maryland, Virginia)
Desired Qualifications
- Current credentialing with major insurance providers
- Minimum of 2 years of direct adult primary care experience
- Excellent bedside manner with a genuine desire to serve, heal, and personalize care
- Ability to work independently and collaboratively with a small, agile team
- Strong communication and organizational skills
- Commitment to delivering timely, compassionate care
