Primary Care Nurse- NE Jackson

Partner closely with Primary Care Providers to support care coordination and follow-up for patients and families. Perform patient assessments, collect data through history-taking and physical evaluations, and conduct direct care including vitals, medication administration, and monitoring. Assist with treatment procedures, monitor patient status, and triage concerns to ensure timely, appropriate care. Provide education and counseling on diagnoses and care plans, while documenting all interactions accurately in the electronic medical record system. Schedule is Monday through Friday, 8:00am to 5:00pm. Requires current Mississippi RN or LPN licensure and minimum two years of clinical experience.
