Primary Care APRN

Primary Care Nurse Practitioner position providing medication management and behavioral health services for child, adolescent, and adult clients with SMI/SED/SUD. Rotates across rural Williamstown, Carrollton, and Falmouth offices with Friday shifts at Covington location (on-site, M-F 8:30–5, no evenings/weekends). Responsibilities include conducting assessments, developing service plans, prescribing medications per policies, coordinating with Lead Medical Officer, maintaining HIPAA-compliant documentation, and participating in multidisciplinary team meetings. Requires Master’s degree in nursing, experience in primary care and behavioral health, DEA (highly desirable), and ability to work independently within an integrated care setting.
