Ambulatory Primary Care RN providing clinical nursing care in the outpatient setting, offering care coordination, patient education, triage, and disease management. Responsibilities include comprehensive patient assessments, development and evaluation of care plans, monitoring clinical status and medication adherence, coordinating with physicians and specialists, delivering patient and family education, triage and escalation as needed, medication management support, and ensuring HIPAA-compliant documentation and high-quality cardiovascular and population health outcomes.
