Primary Care Advanced Practice Provider

Conduct advanced nursing practice for assigned patient populations, including collecting health data, developing care plans, and coordinating clinical services. Deliver comprehensive care to new and established patients through history, physical exams, and treatment plans while ordering and interpreting diagnostic studies. Provide personalized education on self-management and medications, maintain prescriptive authority requirements, and ensure all procedures meet quality standards and state regulations. Participate in continuing education, clinical meetings, and the call schedule to support staff and maintain system-wide communication.
