Job Summary
Manage each assigned Program Participant's prevocational plan of service and required documentation. Assist individuals to develop work skills and behaviors for maximum potential through daily interventions and cooperation with staff. Complete ISP plans, Habilitative Plans, monthly notes, and six-month reviews in Therap, while verifying attendance and monitoring daily documentation. Submit monthly billing documentation and coordinate annual community assessments. Identify barriers to progress and assist with access to community services. Maintain communication with families, care managers, and residential staff. Some travel between Jefferson and St. Lawrence Counties may be required.
Required Qualifications
- Bachelor's Degree in Human Service Field
- One-year experience working with the Developmentally Disabled population or in a vocational setting
- Must have a valid NYS Driver's License with an acceptable driving history per the agency's insurance requirements
- Authorized to work in the United States
- Must be able to lift, push, or pull in excess of 30 pounds
- Must be able to stand
- Must be able to walk
- Must be able to use hands to finger, handle or feel
- Must be able to reach with hands and arms
- Must be able to talk
- Must be able to hear
- Must be able to regularly perform the essential functions of this job
