Job Summary
The Prevention & Wellness Nurse Practitioner contributes to the care of the Medicare population by supporting wellness visits and improving documentation of chronic conditions. This role involves quality improvement efforts, providing comprehensive patient care across diverse populations, and expanding community rapport. The nurse practitioner is responsible for managing daily clinical tasks, communicating with health agencies, and teaching team members and students. Qualifications include a Bachelor's Degree, completion of an accredited Nurse Practitioner Program, and Colorado licensure. One-year ambulatory healthcare experience and proficiency in Spanish are preferred.
Required Qualifications
- Bachelor’s Degree
- Graduate of an accredited Nurse Practitioner Program
- Licensed in the State of Colorado
- Eligible for DEA licensure
Desired Qualifications
- One year of experience in ambulatory health care preferred
- Sensitivity to low income, ethnic minority communities preferred
- Proficiency in the Spanish language
- Computer proficiency – EMR experience preferred
- Ability to flourish in a team system
