Job Summary

Prevention Navigator role within Housing Works’ Biomedical Prevention Services to meet clients where they are, providing HIV prevention and testing services, rapid HIV/Hep C/STI testing, telehealth support, referrals to internal/external services, and engagement to maintain care. Responsibilities include delivering culturally competent HIV prevention education, assisting clients with assessments and benefits evaluation, coordinating referrals, facilitating rapid testing with sensitive results disclosure, supporting home-based telehealth testing, assisting with enrollment in related services (psychiatry, behavioral health, harm reduction, care management), proactive client retention through reminders and follow-ups, and documenting interactions in electronic health records. Requires ability to work remotely with appropriate data security, strong communication, confidentiality, and collaboration within a cross-functional team, with some evenings/weekends as needed. Education preferred: Bachelor’s degree in a health field or equivalent experience. Must be able to work independently and in team settings and to maintain HIPAA compliance.
