Job Summary

Conduct family assessments and provide supportive casework counseling in homes to empower parents with tools ensuring children's safety and permanency. Deliver effective case management by scheduling counseling sessions, coordinating medical and dental exams, and tracking services while maintaining documentation and confidentiality. Engage youth and families biweekly in community settings to strengthen parenting skills, address barriers to stability, and facilitate reunification for those in kinship or foster care. Develop alternative placement resources and complete discharge summaries eight weeks prior to release. Maintain relationships with schools and community agencies to support educational continuity and resource connections. Requires flexible schedule with variable hours, on-call rotation, and travel.

Required Qualifications

  • Minimum Bachelor's degree in human services or related field
  • Minimum of 1 year of experience working with at-risk youth & families and case management
  • Ability to work a flexible schedule with variable hours including day, evening, and subject to call hours
  • Valid Driver's License with acceptable driving history to the Agency, and a reliable vehicle
  • Ability to work with economically and culturally diverse population
  • Ability to handle crisis intervention in solution focused manner
  • Communication skills including written, verbal and listening skills
