Job Summary

Conduct family assessments and implement treatment modalities to support youth and parents facing academic failure, truancy, or behavioral concerns. Deliver case management services by scheduling counseling, coordinating medical and dental exams, and maintaining comprehensive documentation. Perform at least two monthly home and community contacts to build relationships, ensure child safety, and connect families with school and resource agencies. Complete discharge summaries eight weeks prior to placement transitions and manage alternative resources for youth. Work flexible hours including evenings and weekends as needed, with on-call rotation for immediate crisis intervention. Requires valid driver's license and ability to travel for visits.

Required Qualifications

  • Minimum Bachelor's degree in human services or related field
  • Minimum of 1 year of experience working with at-risk youth & families and case management
  • Ability to work a flexible schedule with variable hours including day, evening, and subject to call hours
  • Required to be on call in collaboration with a Program Coordinator
  • Must have a Valid Driver's License with acceptable driving history to the Agency
  • Reliable vehicle
  • Travel is required
  • Ability to coordinate and plan travel for visits and appointments in efficient manner
  • Ability to work with economically and culturally diverse population
  • Ability to handle crisis intervention in solution focused manner
  • Communication skills including written, verbal and listening skills
