Job Summary
Conduct home visits to assess patient needs and provide necessary care in Westborough, Hopkinton, and Holliston. Utilize Case Management, Oasis E documentation, Time Management, and EMR systems to ensure accurate patient progress tracking. Collaborate with patients, families, and interdisciplinary team members to develop and implement care plans. Requires a valid MA RN license, minimum one year of med/surg experience, and willingness to travel within the designated territory. Full-time schedules include three to five days, 8a-2p or 8a-4:30p, Monday through Friday.
Required Qualifications
- Valid MA RN License in good standing
- Minimum of 1 year of med/surg experience
