Job Summary
The Patient Navigator identifies people who are HIV+, HepC+, or at risk and not in care, and reengages them in medical services through outreach methods such as calls, texts, letters, and home visits. The role provides accurate information about living with HIV/HepC or taking PrEP, assists with enrollment in medical care and benefits programs, accompanies patients to appointments, delivers informal counseling and support, links clients to health care and social services, facilitates communication with health systems, and plans patient education groups. The position requires local travel, bilingual capability (English/Spanish strongly preferred), and strong verbal/written communication for group presentations. A minimum of two years in health, social or community services is preferred, with a High School diploma or equivalent required.
