Prenatal Case Manager

Job Summary
Conduct initial meetings and follow-up visits with prenatal patients to identify barriers preventing healthy outcomes. Maintain active problem lists, track referrals, and update care plans while documenting interventions in the electronic medical record. Assist patients with transportation, translation, form completion, and applications for charity care, Medicaid, and WIC benefits. Facilitate referrals to behavioral health and community resources, conduct Spanish-language classes, and educate patients on breast pump acquisition. Act as an advocate for patients with community organizations and ensure accurate PRAspect file entry for state compliance.
Required Qualifications
- Bilingual English/Spanish
- Minimum Bachelor's Degree
- Minimum two years experience in a clinical setting
- Experience working with Medical Insurance- Medicaid, HMO, ACA
- Experience with PRAspect for Medicaid payments and GEMS Authorizations
- Experience working with community service agencies
- Knowledge of Microsoft Office, Excel and use of electronic medical records
- Some travel may be required between sites
- Valid NJ State Driver's license in good standing
