Job Summary
Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation and optimal treatment outcomes as part of the comprehensive patient assessment. Collaborate with the interdisciplinary team to develop care plans that identify effective interventions for meeting rehabilitation, treatment goals, and improving quality of life. Provide psychosocial support and interventions to address non-adherence, quality outcome, and quality of life concerns based on acuity level. Deliver educational and goal-directed counseling to patients seeking transplant, while facilitating referrals to community resources, vocational rehabilitation, and insurance coordinators. Participate in monthly quality review meetings, weekly team huddles, and process improvement activities to enhance patient satisfaction and achieve facility quality goals. Maintain current knowledge of local rehabilitation programs and adhere to social work policy and documentation standards.
Required Qualifications
- Masters in Social Work
- state temporary license
- Registered Social Worker Clinical Intern
- Associated Social Work license
- Provisional LCSW License
- 0 – 2 years' related experience
- Obtains necessary state licensure to work independently without supervision within the first two (2) years or as required by state guidelines
- Travel required (if multiple facilities)
