Job Summary
Assess patients' psychosocial status and strengths to inform care planning and rehabilitation goals for in-center and home dialysis patients. Provide supportive counseling, education on chronic kidney disease, and referrals to community resources, including transplant waitlist management and vocational rehabilitation. Collaborate with the interdisciplinary team to address non-adherence, review quality indicators, and participate in monthly quality review meetings. Liaise with insurance coordinators regarding payment issues and patient rights. Assist with staff hiring decisions, provide psychosocial training, and contribute to weekly team huddles. This entry-level MSW role supports the FMCNA commitment to quality indicators and patient satisfaction.
Required Qualifications
- Masters in Social Work
- state temporary license
- Registered Social Worker Clinical Intern
- Associated Social Work license
- Provisional LCSW License
- 0 – 2 years' related experience
- Travel required (if multiple facilities)
