Prelicensed Social Worker - Part time

Assess patients' psychosocial status, strengths, and areas of need affecting rehabilitation as part of the interdisciplinary team. Participate in care planning to identify interventions that improve quality of life and treatment outcomes. Provide supportive counseling services, educational goal-directed counseling for transplant seekers, and referrals to community resources including vocational rehabilitation. Monitor patient adjustments to dialysis, address non-adherence, and facilitate discussions on advance directives and DNR status with the healthcare team. Collaborate with insurance coordinators to address coverage issues and educate patients on payment options. Maintain updated patient resource lists, assist with hiring processes, and contribute to weekly team huddles.
